The cathedral and its extensive churchyard face Via Rossini. The early Christian cathedral of Pesaro (5th. century) has now vanished. It stood within the perimeter of the Ancient Roman city, on the same site as the present cathedral. During the Middle Ages the church was dedicated to St. Terence, martyr and protector of Pesaro and first bishop of the city in the 3rd. century. The belfry was destroyed by Cesare Borgia in 1503, thereby causing the presbytery and abs to be rebuilt and enlarged.

A further radical restructuring process was initiated by the architect Giambattista Carducci In 1865. This was the occasion on which the sensational discovery was made of a large mosaic (ca 900 sq. m.) covering the entire cathedral floor. Thus, more than a metre below the surface, a basilica came to light, identified by critics as the early Christian cathedral. The mosaic was covered with a new floor. The original mosaic floor  was uncovered to view in 1945 - ’50. The date of the mosaic, excluding some medieval retouches, can be set at the middle of the 6th. century A.D. Recuperation work to restore it to view began in 1990.

The cathedral was completely restructured during the nineteenth century, leaving the 12th. century façade more or less intact, the only relatively complete example of Romanesque architecture in Pesaro. The 14th. century portal of white stone is flanked one on either side by 12th. to 13th. century lions, probably emblems of the Malatesti family. The entire cathedral is on a Latin cross plan with three aisles, seven altars and three chapels.
